Activities of Daily Living (ADL) Assessments - Home Visits
Mend can provide ADL supports to Allied Health professional and support funding requests.
Mend can assist with increasing the independence of your patients following discharge from hospitals, after an accident or for adjustments for elderly patients. Reimbursements are available under Medicare if a referral is made under a Mental Health Care Plan.
Home Modifications
Mend can recommend home modifications and recommendations about equipment required to increase independence in your home or workplace. Supporting reports from our Allied Health professional can be provided to support funding needs.
Driving Assessments
If your GP or the RMS request that you have a Driving Assessment for medical or age related reasons, Mend has specially trained registered occupational therapists to conduct this assessment. The assessment takes 2-3 hours and involves an off and on road assessment. The on road driving assessment is with a d disability trained driving instructor. The off road assessment involves an interview, a physical screen, cognitive testing and a brief test of road laws. A report is supplied to the RMS and a copy is sent to the client.
